Sleep Trouble in Menopause — What's Really Happening at Night

"I fall asleep fine, but I wake up at three and can't get back to sleep" — this sentence comes up strikingly often during perimenopause. Declining progesterone plays a central role here: progesterone has a calming effect on the nervous system, and as it decreases, sleep overall becomes lighter and more easily disrupted.

Another, often overlooked factor: nighttime fluctuations in blood sugar. If glucose levels drop significantly during the night, the body releases counter-regulatory hormones like adrenaline and cortisol to raise them again — and this very hormone surge can cause waking, often accompanied by a brief sensation of heat or a racing heart that's easily mistaken for a classic hot flash.

This becomes visible in a sensor trace: does the value drop in the hours before nighttime waking? Does dinner play a role — say, a late, carbohydrate-heavy snack that first raises blood sugar and then, hours later, lets it fall all the more steeply? Or evening alcohol, which keeps the liver from adequately releasing sugar overnight?

Good sleep has many levers, and blood sugar is just one of them — room temperature, screen time, and stress belong on that list too. But precisely because this one lever can be measured objectively, it's worth a look before settling for "that's just how it is in menopause."
